Position Title: Positive Health Case Manager
Reports to: Director of Case Management
Status: Full Time, Exempt from overtime
Salary: $50,000-$60,000/year

SUMMARY

This Positive Health Manager will support the sustainability of the OUTMemphis mission to provide quality case management services to the LGBTQ+ community of the MidSouth, specifically adults living with HIV. This Case Manager position reports to the Director of Case Management. The Positive Health Case Manager will also frequently consult with the Director of Community Health. The Positive Health Case Manager will build and maintain relationships with local agencies also serving individuals living with HIV. Through guided 1 on 1 support, this position provides case management, referrals, and coordination of supportive services for participants on their caseload. The Positive Health Case Manager will collaborate with the OUTMemphis Case Management team to track data and provide programs and services, in compliance with regulations, grant requirements, best practices, and in line with our mission and values.


Roles & Responsibilities 

Case Management and Participant Services

  • Provide case management and general support to participants, inclusive of screening and assessing participants i.e. intakes; creating, implementing and monitoring individual service plans; crisis intervention; connecting participants to health, mental health, employment, housing and other community resources.

  • Prepares and implements case coordination plans by conducting assessments for participants referred, assessing needs and developing case management plans to assist participants

  • Maintain a case load of up to 70 participants monthly for case management needs

  • Provide home visitation, transportation coordination, and services across all OUTMemphis facilities.

Crisis Intervention, Outreach and Mental Health Support

  • Provide crisis intervention, referrals for short term mental health services and psychosocial case management as needed and appropriate.

  • Obtain all OUTMemphis health screening certification trainings as mandated for all staff

  • Assist in outreach and education for communities at the highest risk for comorbid health concerns related to HIV, inclusive of collaborating with our communications team.

Program Administration, Training and Facility Operations

  • Manage related programs budget including the input of receipts for documentation of program related expenses.

  • Maintain accurate, current, and confidential records for case management participants in conjunction with any required CRMs and local and federal laws and regulations, and all internal reporting requirements.

  • Participate in โ€œAll Staffโ€ trainings and events as detailed on the internal staff calendar.

  • Collaborate with the OUTMemphis Staff on the day-to-day operations of all OUTMemphis facilities as a staff manager, providing a on-site ethical guidance to preserve a welcoming environment for all OUTMemphis participants


Core Competencies 

  • Bachelor's degree in Social Work, or related health and human services field with licensure preferred. Candidates with lived experience of social service engagement and navigating social service systems should also apply.

  • Excellent computer skills, including work experience with data entry, database operations, spreadsheets, and other software systems.

  • At least 2 years of professional experience (or comparable volunteer experience) with participant-facing services or programs

  • Strong verbal and written communication skills, strong organizational skills, conflict resolution skills, and interpersonal skills.

  • Ability to manage multiple projects with demanding and competing deadlines and the ability to maintain quality work in a diverse, fast-paced, stressful, and changing environment.

  • Demonstrated ability to work effectively with people of diverse backgrounds including age, ability, gender identity & expression, sexual orientation, education, and ethnicity, in a diverse environment.

  • Knowledgeable of black and brown queer and transgender issues in the Mid-South, including HIV, homelessness, over-incarceration, sex work, family conflict, and other experiences.

  • Believes that lesbian, gay, bisexual, gender non confirming, non-binary, and transgender individuals should live in a world where everyone has equal rights, and are safe, respected, and celebrated.

  • LGBTQ+ people of color with working knowledge of the South are especially encouraged to apply.


This position primarily works regular business hours but also supports OUTMemphisโ€™s walk-in hours (12pm-9pm, Tuesday-Saturday) and outreach programming, which requires evening and weekend hours throughout the year. Your final schedule will be assigned by your supervisor.
